We prefer meeting with our patients in person whenever possible! However, if you are unable to meet, we offer Telehealth visits through Spruce!

Do you accept insurance?
We accept numerous insurance policies. To confirm that your provider will cover a visit, please call our office at 435.634.8848

Where is your office located?
We are located in St. George, Utah at 437 South Bluff Street in Suite #302. Head in the front door and take the elevator or stairs to the third floor.
You'll find our office right next door to the elevator.

How do I schedule a first appointment?
Fill out our new patient intake form and one of our team members will contact you.
